Title: Consider recommendations regarding a First Amendment to the Quest Diagnostics Professional Services Agreement, as follows: a) Approve and authorize the Chair to execute a First Amendment to the Agreement for Services of Independent Contractor Quest Diagnostics to continue providing clinical laboratory services for the Public Health Department, to expand the Scope of Work to include additional testing services for disease control and prevention and sexual assault response, to extend the period of performance an additional year with a revised period of performance from July 1, 2021 through June 30, 2023, and to increase the amount by $350,000.00 for a revised total amount not to exceed $700,000.00; and b) Determine that the proposed action does not constitute a "Project" within the meaning of California Environmental Quality Act (CEQA), pursuant to Section 15378(b)(4) of the CEQA Guidelines, because the action consists of the creation of a government funding mechanism or other gover...

Title

Consider recommendations regarding a First Amendment to the Quest Diagnostics Professional Services Agreement, as follows:

 

a) Approve and authorize the Chair to execute a First Amendment to the Agreement for Services of Independent Contractor Quest Diagnostics to continue providing clinical laboratory services for the Public Health Department, to expand the Scope of Work to include additional testing services for disease control and prevention and sexual assault response, to extend the period of performance an additional year with a revised period of performance from  July 1, 2021 through June 30, 2023, and to increase the amount by $350,000.00 for a revised total amount not to exceed $700,000.00; and

 

b) Determine that the proposed action does not constitute a “Project” within the meaning of California Environmental Quality Act (CEQA), pursuant to Section 15378(b)(4) of the CEQA Guidelines, because the action consists of the creation of a government funding mechanism or other government fiscal activities, which do not involve any commitment to any specific project which may result in a potentially significant impact on the environment.
