Title: Consider recommendations regarding Immunization Local Assistance Revenue Grant Agreement No. 15-10451, as follows: a) Approve, ratify, and authorize the Public Health Director to execute Grant Agreement No. 15-10451 for the Immunization Local Assistance Grant with the California Department of Public Health Immunization Branch to support vaccination activities for the period of July 1, 2015 through June 30, 2017 in the amount of $165,525.00 annual revenue with a total revenue amount of $331,050.00; b) Acknowledge that approval, ratification, and authorization of Grant No. 15-10451 replaces and cancels Immunization Action Project Revenue Agreement 13-20345 that had a period of performance from July 1, 2013 to June 30, 2017; and c) Determine that these activities are exempt from California Environmental Quality Act (CEQA) review per CEQA Guideline Section 15378(b)(4), since the recommended actions are government fiscal activities which do not involve commitment to any specific pr...

Consider recommendations regarding Immunization Local Assistance Revenue Grant Agreement No. 15-10451, as follows:

 

a) Approve, ratify, and authorize the Public Health Director to execute Grant Agreement No. 15-10451 for the Immunization Local Assistance Grant with the California Department of Public Health Immunization Branch to support vaccination activities for the period of July 1, 2015 through June 30, 2017 in the amount of $165,525.00 annual revenue with a total revenue amount of $331,050.00;

 

b) Acknowledge that approval, ratification, and authorization of Grant No. 15-10451 replaces and cancels Immunization Action Project Revenue Agreement 13-20345 that had a period of performance from July 1, 2013 to June 30, 2017; and
