Title: Consider recommendations regarding Cachuma Reservoir Re-Roof, Project No. 8437A (General Services Project No. 8588) Construction Contract Amendments No. 5 and No. 6, Third District, as follows: a) Approve and authorize the Chair to execute Amendment No. 5 to the Contract with Hanly General Engineering Corp. Title
Consider recommendations regarding Cachuma Reservoir Re-Roof, Project No. 8437A (General Services Project No. 8588) Construction Contract Amendments No. 5 and No. 6, Third District, as follows:
 
a) Approve and authorize the Chair to execute Amendment No. 5 to the Contract with Hanly General Engineering Corp. ("Hanly") for the Cachuma Reservoir Re-Roof project in the amount of $9,698.00 upon return of the Contractor's executed Contract documents and the review and approval of County Counsel, Auditor-Controller and Risk Manager or their authorized representatives;
 
b) Approve and authorize the Chair to execute Amendment No. 6 to the Contract with Hanly for the Cachuma Reservoir Re-Roof project in the amount of $191,700.00 upon return of the Contractor's executed contract documents and the review and approval of County Counsel, Auditor-Controller and Risk Manager or their authorized representatives for a total amended Contract amount of $517,256.00;
 
c) Re-authorize the Director of General Services or designee to approve Change Orders for a total amount not to exceed $19,708.00 for this Contract; and
 
d) Find that pursuant to the California Environmental Quality Act (CEQA) Guidelines Section 15162, no substantial changes are proposed, and no new information of substantial importance has come to light regarding environmental effects of the project or of the sufficiency or feasibility of mitigation measures, and therefore these actions are within the scope of the project covered by the Mitigated Negative Declaration 09NGD-00000-00024 and monitoring program approved and adopted by the Board for this project on April 19, 2011, and determine that therefore no new environmental document is required.
