Title: Consider recommendations regarding Targeted Case Management Provider Participation with the California Department of Health Care Services, as follows: a) Approve and authorize the Public Health Department Director to execute an Amendment to Agreement No. 42-1318, the Medi-Cal Targeted Case Management Provider Participation Agreement with the State of California, Department of Health Care Services for services allowable for Federal Financial Participation, anticipated to be $1,250,000.00 for the 5-year period July 1, 2013 through June 30, 2018; b) Authorize the Public Health Department Director to execute any other documents necessary to carry out Targeted Case Management Provider activities under Agreement No. 42-1318, including the Certification Form CCC-307, the Certification Regarding Lobbying, and any future amendments that do not change the amount or term of the Agreement; and c) Determine that the above actions are organizational and administrative activities of governme...

a) Approve and authorize the Public Health Department Director to execute an Amendment to Agreement No. 42-1318, the Medi-Cal Targeted Case Management Provider Participation Agreement with the State of California, Department of Health Care Services for services allowable for Federal Financial Participation, anticipated to be $1,250,000.00 for the 5-year period July 1, 2013 through June 30, 2018;
