Title: Consider recommendations regarding a Second Amendment to Maxim Healthcare Services, Inc. for Fiscal Year (FY) 2016-2017, as follows: a) Approve and authorize the Chair to execute a Second Amendment to the Agreement for Services of Independent Contractor with Maxim Healthcare Services, Inc. (not a local vendor) to add an hourly position for an Occupational/Physical Therapist, with no change to the total contract maximum amount through June 30, 2016; b) Approve and authorize the Behavioral Wellness Director to amend the Exhibit A (Statement of Work) and Exhibit B-1 (Schedule of Fees) to add hourly positions and approve rate changes as needed, without returning to the Board with a separate amendment, so as long as the total contract maximum is unchanged; and c) Determine that these activities are exempt from California Environmental Quality Act (CEQA) review per CEQA Guidelines Section 15378(b)(4) since the recommended actions are government fiscal activities which do not involve comm...

Title

Consider recommendations regarding a Second Amendment to Maxim Healthcare Services, Inc.  for Fiscal Year (FY) 2016-2017, as follows:

 

a) Approve and authorize the Chair to execute a Second Amendment to the Agreement for Services of Independent Contractor with Maxim Healthcare Services, Inc. (not a local vendor) to add an hourly position for an Occupational/Physical Therapist, with no change to the total contract maximum amount through June 30, 2016;
